Please see attached letter. A few days ago, I was advised by [name] at [XXX] Medical Center that although QuantumLeap was told the procedure would cost $x,xxx.xx – they “adjusted” the amount to three times that amount “for the insurance company.”

It appears that [XXX] Medical Center exaggerated the amount of the procedure in an attempt to bilk, overcharge or defraud the insurance company and because the insurance policy only provided a $x,xxx limit, they now want to pass on the fraudulent amount to my client, QuantumLeap.

Demand is hereby made to correct this attempted act of fraud. Please advise in writing that the invoice for an additional $xxxx.xx to QuantumLeap is deleted and that no money is owed. In addition, please advise the collection agency of the matter and take all efforts necessary to repair my client’s credit. If this matter is not resolved in 10 days, we will file a lawsuit and let the courts handle this act of fraud. Thank you.
