Elvis had a long history of taking caffeine pills and uppers to get thru shows (imagine the schedule playing sold out Hilton shows EVERY NIGHT) then taking sleeping pills to sleep.

He'd take laxatives to speed up the impact of the drugs...but those can dry out the colon eventually, leading to hard non moving stools....and a severe case of constipation.

So Elvis died on night of a heart attack from downers, constipation and the stress on his heart of trying to push a rock hard stool...

But the phrase grandson refers to some guidance in rearing by a grandfather. This is a child whom Elvis never met or knew of, he is only a grandson in name, not in relation.
