The cases are not fake. They have an endless pool of small claims cases to choose from. If they are not compelling viewing they just don't get aired. Judge Judy is an enormous franchise and it would be absurd to threaten the very existence of this money-making juggernaut to arbitrarily sprinkle in staged cases. It's like when people say the lottery is fixed. It simply doesn't have to be because it will make oodles of money on its own and the only way to derail it is to do something stupid...like fix it.
