There were probably two operatives involved, dressed in ordinary clothing and could easily slip away during the confusion even with all the cameras on the property. The NRA have to keep a small circle for the conspiracy, maybe a couple well placed govt officials, a couple of law enforcement officials that can help steer the investigation if needed.

In every large organization there is at least a couple aggressive, out of the box, forward thinking members that are willing to do what it takes regardless of the costs. It may seem crazy to most normal people, but there are zealots out there and some in positions of influence. As far as leakers, that's why the need to keep a small circle of like minded individuals with the common goal. The operators themselves may have no idea that this was an operation from a fringe NRA group, just mercenaries paid extremely well to do a job no questions asked. It could have been hired foreign operators thinking the goal of the attacks were foreign based and political.
