well, i would number one just forget about Fish's career that you remember the high points,
because he's older and is going to be rusty. He also has mental issues, perhaps those are
overcome but it seems unreasonable to expect him to beat Harrison the way Harrison is in great form.
I watched Fish in doubles, he was pretty good, but this is outdoors, and singles, and more for real.

I was only teasing, but all jokes aside, it can be a debilitating form of mental illness, i.e., Panic Disorder, regardless of the etiology.
