Best way to stop spam/junk email?

Noticing a ton either from online casinos/books, penis enlargement sites, etc, and the latter I've no reason to be on mailing lists believe me.

I have an integrated spam filter on both AOL/Yahoo, but it doesn't zap them all. How do you clean up your email?

For example:

Anyone not in my addressbook automatically ends up in my junk folder, when something is in my inbox it's 100% not spam.
