I used to have no problem having most of my roll lying around in neteller or say pinnacle but those days are over (well, especially since neither of those places want my action, as an American, but that is a different discussion) ... I don't trust anyone with a significant amount of my money anymore...
Right now I keep just what I need online in case I run bad but I feel better having my money in the bank or my safe deposit box and will reload as needed... I play primarily poker this days and basically I try to keep about 10 full buyins online... (I multi-table 5-8 games at a time...) Anytime I get below 5 buyins or over say 20 buyins I usually deposit or withdraw...
On the other hand I can see people who play bigger having trouble moving large amounts around these days and being forced to keeping larger balances... Once you get to the point where you need to deposit over a couple thousand at a time I can see even infrequent deposits and withdraws being a problem...

Depends, nothing set in stone. Through the course of a year balances will fluctuate, it really comes down to leaving yourself a sufficient amount per book.

Seriously all my money stays on line in various books or moneybookers. Depending on the book I will withdraw in 1K chunks (I do that with royal). When I withdrew 5K from SIA it was enough to have them limit me. The same thing happened last year at bowmans. From now on I withdraw in 2K chunks.
