You should always tip, they don't make a lot of money and depend on your 10-20% tip to survive. Plus, if you order regularly from there, you don't want to get the rep of a non-tipper for obvious reasons.

I order a meat lovers from Pappa Johns
I also gave him a couple of dollars and he seemed like that there was a tip already included(kept thanking me and told me he hopes I enjoyed the pizza) It was good !!

you can choose papa johns or dominos
double tipping is ok but i would only do that if the service is good
there have been times i have waited an hour for a delivered pizza, then the guy shows up with an attitude like "here's your goddam pizza where's my tip" - that to me is deserving of NO tip and any pizza delivery guy that would mess with someone's food should not only be fired they should be arrested
