EDIT: It's strange because I get to the bookmaker screen just fine and everything looks great, it tells me my device (envtouch) and has all the info about it, then has the compatible list below it for which I'm not listed. Can you clarify that list is not all inclusive or if I can access the site is that good enough?

When creating the play-money account at http://m.bookmaker.com, under Step 3 there is a box titled 'bonus code'.

The booby prize is the last place prize, so in a sense yes it's the fourth prize given away.

The first person to lose their entire 5,000 play-money balance wins the booby prize.

There aren't any guarantees that you will be able to successfully use the platform and participate, but you're free to try. All I can say for certain is the devices listed were specifically used in the creation process of the software, non-listed devices may not work.

Did you attempt to create the account at http://m.bookmaker.com using your mobile device? If so, confirm that the e-mail you are using isn't one already associated with a Bookmaker account, should be accepted normally.

No, you must create a test account at http://m.bookmaker.com. These accounts will have play-money distributed to them on Monday as listed in above details, after the contest these accounts will be deleted by Bookmaker, it's just for test purposes and to give feedback.

No need to PM the acct, but use a valid e-mail address so that Bookmaker can contact you after the contest ends.

I'd suggest using the same phone # etc as registered to the Bookmaker account you have that your potential winnings will be deposited to. Keep in mind to approach as a feedback contest as prizes can be sent out just for providing helpful comments or finding a potential bug.

Some might have the strategy to shoot for the $50 instead of the higher prizes, but the booby prize would be determined by time-stamp that the ticket is made so it'd still be a coin-flip on who empties the acct first.

Correct. The accounts registered for http://m.bookmaker.com are basically just test accounts for their new platform, it won't be considered having a 'second' Bookmaker acct. Once the contest is confirmed as over Bookmaker will erase the play-money accts from the system and you won't be able to use them again.
