I'm guessing that the purpose of the form is to prove that their customers are not laundering money. What they will do with the form is beyond me.

Has a sportsbook actually asked you to fill out such a form or have you run out of questions to ask?

Offshore banks will ask for that sort of documentation.

If a sportsbook makes such a request dump them.

And books are not doing it?

I guess that is the reason they are all offshore and with so many regulations within the US ... no?

There are lots of books that don't require that documentation. Why should you waste your time fullfilling such an intrusive request unless the book offers something that no other book offers you?

Anyway: What book exactly asks for that?

Or is it just an academic question?

                  Source of income: Pimp

What an absurd fukkin request. They either want your business or they don't.

it is a simple form asking your name, address, account number, id number and what is your source of deposit money. you can list things like savings, investing, gambling profit, inheritance etc. then you sign it and fax it to them.

not sure what will they do if you list pimping as a source of income. i think that falls under the catagory of money laudering (sex trade)

I've never heard of anyone being asked for that. Offshore sportsbooks do not report anything to the US or other countries.

When regulation does finally come a player will have to fill out an application similiar to a stock brokerage application and disclose income, expierence, net worth, ect.

Never been asked for that, and I would be very cautions regarding filling out such a form for a book based outside the US for the reasons SBRJohn indicated.
