five dimes
betjm
legendz

basically every book out there. though i guess it depends on how you deposit. stay away from credit cards.

fedex a cashiers check, western union

i don't think any of the sportsbook.com books require any forms. they don't seem to be paying anyone either though. i'd stay away.

Find a book you like and email them and ask how you can deposit. www.betjamaica.com would probably be a good choice.
