1. When did you start gambling?
2. Do you have any relitives that gamble?
3. When did you know you have a gambling problem?
4. Do you have any other addictions? (smoking, drugs, drinking, etc)
5. If you could go back and change anything prior to gambling, would you?
6. have you even tried to seek help for your gambling problem?
7. does society judge you for your gambling problem?
8. do family and friends know you gamble? does it affect them?
9. what have been the consequences of your gambling?
10. what are the ingrediants that make up a problem gambler?
