I also immediately thought of Colorado in College. Terrible year after year, they never improve.
I would also add the Panthers, Jags, Browns, Raiders and Chargers in Pro Football.
The Chargers gave away Drew Brees, Michael Turner, Darren Sproles, Cromartie and Vincent Jackson. Gave away their pieces for a chance at Super Bowl.

LSU always seem to have dumb penalties, bad clock management, and terrible QB play. They counter it by burying a giant horseshoe under the stadium.

Carolina Panthers no doubt. That's at least the 3rd game they completely let unravel in the 4th quarter.
