    Predicting An NBA "Turned Off" MoneyLine
    Is there some general rule of thumb, that says something like
    "Historically, if the spread is above +/- x.x then the moneyline is typical turned off"?

    It depends on the book. Some books (pinny, 5 dimes) will always place the moneyline. Some others are very conservative, does not offer anything opening at +400 or higher (in such cases, they keep offering it even if climbs higher). And the rest have a mixed approach,does not like to offer very high lines, but often do it in high profile games. I know that your question is aimed to NBA, but also works that way for NCAAB, NCAAF and even NFL.
