Thanks for posting those links. I just got the following e-mail from the "good" folks at Bodog...

We have received notification from our Debit Card provider that they are not longer able to process payout requests via Debit Card. Unfortunately, this means that the card you were recently sent is no longer valid, and we encourage you to simply shred and discard it upon receipt.
