I've had it pop up probably 3 times in the last 6 months.

Yep, I got it and filled it out. Then I got it some more.

I've had it nearly everyday for the past week. OK, I've logged in a few times but still.

Keeps popping up when I am trying to get down a bet quickly after logging out.

Never seen it. Is it a pop up window? If so, then maybe that is why. Maybe the Firefox popup blocker is catching it and I dont notice.

I got it twice yesterday and filled one out a few months ago.
