The enforcer rule has slowly diminished over the years, it was a lot better back in the day, when you would send out Semenco or Dale Hunter.

If the ref decides the elbow was deliberate, then it's a major for sure.

Up to the ref but I don't think they review at the game, not sure. The league might had that one down after the fact though if it was something the ref didn't see, especially since there was an injury to raise attention.
