I think that avatars should definitely have to be approved by mods. Otherwise you have gross avatars like no1here's avatar, which makes me sick to look at it. Another idea is to implement a feature that allows you to blank out a user's avatar if you don't like it. The avatar appears blank to you even though other users can see it. This is possible in Pokerstars site for example.
