All I did was set up coinbase account and then set up blockchain account use Blockchain as the middle man between books and coinbase

I kept it simple

1) Open a free wallet at blockchain.info/wallet to receive the funds from the bookmaker. (and receive bitcoin from SBR if you spend your points on that)

2) Open and verify an account at Coinbase.com, where you can send the bitcoin to be swapped for US$ and sent to your bank account.

That's about it really.

You'll need to take care to read the instructions about getting verified at Coinbase. That's about the only mucking around part and could take a few days.
