If it is an efficient market, the 2h total and scoring drop in roughly equal amounts. Sharps pound the unders if the h2 total is too high, and these quickly get into line.

You raise a good point though. If one team has a big lead, the fair h2 total might be 4-5 points lower than what people would expect.

Makes a difference whether the team leading by 21 was the dog or fav.

that would tend to lead to lower 2nd half scores by running the ball i would assume

The unders (NFL) have not done well in those +21 or more fav leading scenarios last 10 years or so, I know, i've been betting them.

NCAA has done better but my criteria for playing them might be different than others.

I will go over 24%. I will go under 26%. If the markets are fair, the 1H over is 50%, then the 2H over is 50% (since it includes all information so far), and the game total is then 99.9% to go over (there can be very strange cases where it won't). Unders are slightly better than overs in general, so I'm guessing we're a little under 25%.
