BetUS was the first book to ever cut me off, I played real stupid there, going huge on tiny soccer leagues and what not and cashing out every week. You definitely don't want to burn your bridge there because they don't accept new US players. Probably one of the top 3 softest books out there.
They've always had suspect ratings, but SBR ratings are worthless IMO. They gave BetIslands an A, they stole my money, they've given Sportsbook.com a D- and I've cashed out well over 6 figures there over years and years.
As far as taxes, taxes are due on winnings no matter what. Whether or not its reported, its still your duty to pay taxes.
There are various levels that banks look at for funds transfers, $2500 and $10000 being the two most common. So, a lot of people try to stay below the $2500 mark, however you can get in trouble there, if you make multiple withdrawals below the threshold you can be accused of Structuring, which is a crime in and of itself.
If you're making a profit from this, just pay your taxes, deduct your trips to vegas and whatnot. Better to not worry about it.
Personally, like I said before, I don't cash out anymore until the book limits me. The balance doesn't matter. If they're not going to pay you, they're not going to pay $1000, or $100,000.
