Can't access CC verifaction form from work

I know Bodog sucks. That said, I do have a small bankroll with them and am looking to payout. I was hoping to fax out the credit verification form here from work but the site is blocked. I was hoping someone could email me the form.

Why does Bodog suck? I've had quick payouts for the past few months. Personally, I really like Bodog as one of my outs.
