    moneybookers help
I am trying to set up an account there, but was wondering a couple things. First, if I make an account there how do I fund it? And then If I use moneybookers to deposit on bookmaker what forms do I need to fill out before I can request a payout back to moneybookers? Seems cool that bookmaker allows payouts to moneybookers once a week, problem is what all needs to be filled out in order to do this?

You can fund moneybookers via bank wire or credit card. Credit card funding is charged a fee of around 2%.
You also won't have to fill out any forms to withdraw anything back to your moneybookers account.

So If i was to open moneybookers and fund it by credit card, then fund bookmaker by moneybookers, I can request the payout once a week by moneybookers and not go through the credit card authorization forms?

You will go through credit card authorization process anyway. It will be done by Moneybookers. The negative thing is that they lock up your account until they process your documents, you will not be able to move funds, the process might take up a week.

If you withdraw funds from Bookmaker using Moneybookers, you can expect the money to arrive in your account in just a couple of hours, without any information requested by Bookmaker.

the good thing with moneybookers is that you do the boring stuff once, instead of once per bookmaker

I highly doubt you can use a us bank account given moneybookers has stayed out of the US market. But I honestly don't know.

Some bookmakers don't give bonuses to mb depositors, check the rules if you want to take part in a promo. The payout time to mb depends on the bookie. F.e.: Paddy Power 1-2 hours, Bet365 2-4 hours, TheGreek 8-12 hours, Bwin 24-72 hours etc...
