The tightest margins I see are pinnacle, betonline, betfair , BAS and Betus all basically $.20 margins up to 180

JJ while this is somehow correct in a way but I look for value/tightest margins before placing a bet in specific matches. The ones you mentioned are all good except BF in terms of odds and commission.

If I take Nakashima+1.5 spread is there a scenario where he can lose and I can still win my bet with the spread? Never bet tennis spreads before.

This is tricky: for the +1.5 games spread to win = Nakashima must win at least one set. If you are not feeling confident then +2.5 games spread is a much better choice, here he doesn't need to win any set he just need to not be broken in any set even if he lose both tiebreaks. like (7-6, 7-6)

BUT im going for Nakashima to win ...so..

yes as i said +1.5 games spread is tricky and risky, Nakashima can win the match and your bet lose. for example he can win the match with only 14 games but conceded 18 games. like (0-6, 7-6, 7-6)
