That does not sound too strange to me.

If they have not asked for ID I would guess that they either think they know who you are without it, or it's not about you personally.

Not a lot you can do but wait for a decision and react then if needed.

Bovada do not have a name for just cancelling winners because they do not want to pay. And $17,000 is not a big balance for them. So if you know they have no reason to penalize you, you probably have a very good chance to still be approved and paid.

Is their a dispute forum I can utilize here, I was under the impression SBR acts as a mediator between Client and Sports books?

Sorry, SBR no longer mediates with offshore books. New owners have moved to mainstream licensed books only.

Bovada has in their terms you can't wager more than 50% of your deposit amount if you take a bonus.

While I've done it before on occasion and they did not care- I'm sure they can pick and choose when they want to enforce it.

If you deposit $500, get $250 bonus. You can't bet more than $250 on anything. Casino, sports, horses, doesn't matter.
