Bitpay's great. They take the middle of the Bid/Ask prices at Bitstamp the second you want to load your card, then charge you a tiny fee -- way less than 1% -- and give you 15 minutes to get the withdrawal started.

At first you'll have to wait for a confirmation before the card's loaded, but after trust is established and they're sure you're not trying to manipulate them based on BTC's price direction, they'll start crediting your card immediately, with zero confirmations.

It's a V I S A debit card, so you can use it everywhere that card's accepted (so basically everywhere) or withdraw cash directly from any ATM. It seems to good to be true, but there's no catch. I've been using Bitpay to pay for everything for over a year.
