1. On a penalty shot if the shot is unsuccessful is the penalty then enforced i.e. must the penalized team send the offending player to the penalty box?
2. Is their a maximum number of players a team can have in the penalty box at one time? I know that the goalie cannot leave the ice due to a penalty. But is it theoretically possible for a team to have all their forwards, center and defense men off the ice serving penalties leaving only the goalie on ice? If so, has this ever happened?

Apparently the answer is no because the situation arose in the Vancouver at Anaheim game 15 minutes after my post. Vancouver awarded penalty shot, missed it and no power play situation ensued. Interesting timing.
