If someone's record has long gaps of inactivity between fights, prior to USADA, they could've been juicing in their off season when they weren't being tested. If someone has cardio that lasts only 2-3 rounds that could be a result of steroid abuse. If someone has to make a strong effort to pace themselves and fight very conservatively not doing much in a round, they might've be juicing.

Whether people was using PED's or not could have some warning signs.

Steroid abuse can damage a person's body and permanently give them bad cardio. It can also shorten their lifespan and give them complications even after they retire from fighting.
